Health Insurance for Baby (Add as Dependent — Employer Insurance)

If you are enrolled in employer social insurance (shakai hoken), you need to register your newborn as a dependent (hifuyosha) under your workplace health plan. The baby can still receive medical care before the health insurance card or Certificate of Qualification (資格確認書) arrives, but apply as soon as possible.

Documents needed

Health Insurance Dependent Change Notification form (obtain from HR/General Affairs dept.)
Baby's My Number (from notification letter or resident record)
Proof of birth (copy of Maternal and Child Health Handbook, or resident certificate extract)

Steps

As soon as possible after birth, inform your company's HR or General Affairs department and request dependent registration

Fill in and submit the 'Health Insurance Dependent Change Notification' form with the baby's name, date of birth, and My Number (form provided by HR)

Your company submits the form to Kyokai-kenpo (Japan Health Insurance Association) or your health insurance union

The baby's health insurance card or Certificate of Qualification (資格確認書) arrives (usually within 1–2 weeks)

Important notes

Apply as soon as possible. The baby can receive care before the card or Certificate of Qualification arrives, but you may need to pay upfront and claim reimbursement later

If the baby visits a doctor before the card or Certificate of Qualification arrives, pay out-of-pocket first, then bring it to the clinic for reimbursement

Register the baby under the parent with the higher income. If both parents have employer insurance, add to the higher earner's plan

Once the health insurance card or Certificate of Qualification (資格確認書) arrives, apply for the Child Medical Expense Subsidy (医療証) at your municipal office

The baby's My Number notification typically arrives several weeks after birth. If you don't have it at the time of application, the dependent registration will still be accepted — notify your company or insurer once the My Number is confirmed
