Child Medical Expense Subsidy (子ども医療費助成)

Your local municipality subsidizes out-of-pocket medical costs for children. Once the baby's health insurance card or Certificate of Qualification (資格確認書) arrives, apply as soon as possible to receive a 'Child Medical Certificate' (Kodomo Iryo-sho). Steps

Once the child's health insurance card or Certificate of Qualification (資格確認書) arrives (or on the same day as birth registration), visit the relevant counter at your municipal office

Fill out and submit the 'Medical Expense Subsidy Application Form'

Receive the Child Medical Certificate (issued immediately or mailed later)

At future medical visits, show both the health insurance card or Certificate of Qualification and the medical certificate to receive the subsidy

Important notes

Coverage age varies by municipality (e.g., free through end of junior high, or free through end of high school). Most municipalities cover both outpatient and inpatient care

Benefits typically start from the month of application — apply as early as possible

You can usually handle this on the same day as birth registration and the child allowance application

If you forget the medical certificate at an appointment, most municipalities allow you to claim reimbursement retroactively (償還払い)
