When moving to a different city or ward, you must withdraw from your current National Health Insurance and re-enroll in your new municipality. Not required if you are covered by employer health insurance.

Qualification Confirmation Card or National Health Insurance card (if you have a valid one)
Residence Card
ID document

Steps

At your current city / ward office during move-out notification, withdraw from National Health Insurance (or bring your Move-Out Certificate to the new office)

At your new city / ward office, enroll in National Health Insurance at the same time as your move-in notification

3

Receive your new Qualification Confirmation Card or eligibility notice (may be mailed to you)

Important notes

Not required if you are covered by employer health insurance (shakai hoken)

Failing to enroll within 14 days may result in a gap in coverage

Insurance premiums may be charged separately by both your old and new municipality
